Job summary

AHN Healthcare@Home in Bridgeville, PA is seeking a Home Health CNA to provide compassionate care to patients in their homes within the Bridgeville area. The role emphasizes autonomy, patient relationships, and meaningful growth in a hospice-oriented home setting.

You will deliver ADL support, monitor conditions, educate families, and document in the EMR while traveling to patient locations within your territory. A dedicated education team supports your development from day one.

Qualifications

• Active CNA certificate in the state you work
• High School Diploma or certificate
• Minimum one year healthcare experience
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
• Provide hands-on care in the home, assisting with activities of daily living (ADLs) while observing and reporting changes in condition to support patient safety and independence
• Educate and support patients and caregivers by reinforcing care plans, promoting healthy routines, and encouraging confidence with daily care tasks
• Respond promptly to patient needs, communicate effectively with the care team, and maintain accurate, timely documentation in the EMR
• Work independently, but never alone
• Travel to patient homes within assigned territory
