AHN Healthcare@Home is seeking a Home Health Certified Nursing Assistant (CNAC) to support patients in Washington, PA and surrounding areas. The role involves collaborating with the care team to manage and deliver individualized care plans and ensuring timely support to patients at home.

The ideal candidate holds a state CNA certification, a high school diploma or equivalent, and at least one year of healthcare experience, with the ability to drive 20% of the day and manage schedules

• Minimum of one year of healthcare experience.
• Current driver’s license and ability to spend approximately 20% of your day driving to and from patient locations.
• Self‑management of time and schedule.
