Job summary

Discovery At Home is seeking a Clinical Support Specialist to assist the clinical team with documentation, scheduling, and EMR systems support in a Texas home health setting. The role requires accuracy, regulatory awareness, and strong communication to ensure high-quality patient care.

Under RN Clinical Manager oversight, the specialist will help maintain compliant records, support field staff, and contribute to QAPI initiatives while leveraging a Texas LVN license and CPR certification.

Qualifications

• 1–2 years of experience in home health, healthcare administration, or medical office setting.
• Strong working knowledge of EMR systems and documentation standards (OASIS, Plans of Care).
• Excellent organizational and communication skills; detail-oriented and able to multitask.

Responsibilities

• Clinical Documentation & EMR Support: Review and audit clinician documentation for accuracy and completeness (e.g., OASIS, Plan of Care, visit notes).
• Coordination & Communication: Support field staff with real-time clinical or scheduling questions and liaise between departments.
• Regulatory & Compliance Support: Monitor compliance with Medicare Conditions of Participation and assist in surveys or audits.
• Training & Technical Support: Provide EMR guidance to new or existing staff and assist with onboarding.
