Job summary

TheraCare Home Health in San Antonio, TX is seeking a Home Health Orders Specialist to manage the clinical documentation lifecycle for physician orders, face-to-face encounters, and Plan of Care certifications. You will liaise with field clinicians, leadership, physicians, and admin teams to ensure timely processing and accurate records in accordance with Medicare requirements.

Responsibilities include generating and tracking orders, communicating with clinicians for signatures, auditing records

Qualifications

  • Minimum 1–2 years administrative experience in a healthcare setting, home health or medical records preferred.
  • High school diploma or equivalent required; Associate’s/Bachelor’s degree in healthcare admin, business, or related field a plus.
  • Familiarity with Medicare guidelines, physician order tracking, and home health documentation workflows.
  • Proficiency with EHR systems, electronic signatures, and Microsoft Office applications (Word/Excel).
  • Strong attention to detail, organization, multitasking, and HIPAA compliance.

Responsibilities

  • Generate, distribute, track, and manage physician orders and Plans of Care from initiation through completion.
  • Coordinate with physicians and PAs/NPs to obtain signatures and ensure regulatory timelines.
  • Audit active patient records to identify missing or unsigned orders and inform leadership.
  • Upload and index signed documents in the EHR and update workflow statuses.
  • Assist intake and admin teams with referrals, insurance verification, and records requests.
  • Create order-tracking, compliance, and delinquency reports to identify gaps.
  • Support workflows that promote timely billing and continuity of care.
  • Maintain patient confidentiality in line with HIPAA and Medicare rules.

Job Summary:

The Home Health Orders Specialist manages the clinical documentation lifecycle for physician orders, face-to-face encounters, and Plan of Care certifications within the home health setting. This role serves as a key liaison between field clinicians, clinical leadership, physicians, and administrative teams to ensure timely processing, tracking, and completion of medical documentation. The Home Health Orders Specialist supports regulatory compliance, care coordination, patient safety, and documentation accuracy by maintaining efficient order-management workflows and monitoring home health records in accordance with Medicare and regulatory requirements.

Qualifications:
  • Minimum of one to two years of administrative experience in a healthcare setting; home health, hospice, or medical records experience preferred.
  • High school diploma or equivalent required; Associate’s or Bachelor’s degree in healthcare administration, business, or a related field is a plus.
  • Familiarity with medical terminology, Medicare guidelines, physician order tracking, and home health documentation workflows.
  • Proficiency with Electronic Health Record (EHR) systems, electronic signature platforms, and Microsoft Office applications, including Word and Excel.
  • Exceptional attention to detail, strong organizational and multitasking skills, effective written and verbal communication skills, and the ability to maintain HIPAA compliance and patient confidentiality.
Essential Functions:
  • Generate, distribute, track, and manage physician orders, including Plans of Care, medication changes, treatment updates, and face-to-face documentation from initiation through final execution.
  • Communicate with physicians, physician assistants, and nurse practitioners to obtain signatures and ensure compliance with required documentation and regulatory timelines.
  • Conduct routine audits of active patient records to identify missing, incomplete, or unsigned orders and communicate discrepancies to clinical leadership and field staff.
  • Upload, index, track, and maintain signed documentation within the Electronic Health Record (EHR) system while updating workflow statuses and documentation records according to agency policies.
  • Assist intake and administrative teams with patient referrals, insurance authorization verification, and medical records requests.
  • Generate order-tracking, compliance, and delinquency reports to identify documentation gaps, compliance risks, and outstanding items requiring follow-up.
  • Support home health documentation workflows that promote timely billing processes, regulatory compliance, and continuity of patient care.
  • Maintain confidentiality of patient information and comply with HIPAA, Medicare, and organizational documentation standards.
