Job summary

Enhabit Home Health & Hospice is seeking an experienced registered nurse to coordinate and oversee home health services, ensuring care aligns with standards. You will liaise with physicians, patients and caregivers to maintain continuity of care and efficient scheduling across the team.

The role requires a valid RN license, CPR, home health experience, and strong communication skills. In-office attendance is essential to supervise, collaborate, and drive quality outcomes.

Qualifications

• Graduate of an approved school of professional nursing.
• Valid clinical license per state requirements or compact license.
• Experience practicing under the professional license.
• Minimum two years of nursing experience within the last 36 months.
• Minimum one year of home health experience.
• Knowledge of federal, state and local laws governing home care operations.
• CPR certification.
• Valid state driver license.
• Automobile liability insurance is maintained as required by law.
• Basic technology skills including mobile devices.

Responsibilities

• Coordinate overall home health services per standards and procedures.
• Review and approve patient information at start of care, recertification, resumption of care, or evaluation visits.
• Facilitate communication among physicians, referral sources, patients, caregivers and staff.
• Schedule patients to field clinicians and ensure care coordination and continuity.
