Puget Sound Home Health in Tumwater, WA is seeking a Home Health Clinical Manager to lead clinical operations in the Tumwater office. The role requires OASIS training and Medicare-certified agency experience, with on-call duties and some direct patient care.

The ideal candidate has at least 2 years of healthcare management, 1 year in home health/hospice, a BSN preferred, and a current BLS. Hybrid schedules are available; relocation not required.

Qualifications

• Minimum 2 years of healthcare management experience.
• At least 1 year of home health/hospice experience with OASIS training.
• BSN degree preferred.
• Current BLS certification.
• Washington state license.
• Experience with Home Care Home Base preferred.
• Knowledge of Medicare COPs, PDGM, VBP.
• Licensed and insured driver.

Responsibilities

• Ensure compliance with home health/hospice laws and regulations.
• Review referrals and manage assignments.
• Oversee patient charts and clinical courses.
• Assist with staff training, hiring, and mentoring.
• Participate in QAPI, OASIS review, and coding workflow.
• Some direct patient care and on-call duties required.
