The Pennant Group, Puget Sound Hospice and Home Health, is seeking a dedicated Home Health Clinical Manager for our Tumwater office in Washington. The role requires experience in a Medicare-certified home health agency with OASIS training.

Responsibilities include ensuring compliance with home health and hospice regulations, reviewing referrals, supervising staff, and contributing to QAPI and coding workflows.

Qualifications

• Minimum 2 years of healthcare management experience.
• At least 1 year of home health/hospice experience with OASIS training.
• Current Washington state license; BLS certification required.

• Oversee patient charts and clinical progress
• Assist with staff training, hiring, and mentoring
