Job summary

Interim HealthCare, the nation’s first home care company, is hiring a Home Health CNA to join our Eau Claire, WI team. You will provide one-on-one care in patients' homes, helping with daily tasks and health monitoring, while building meaningful relationships with families.

Qualifications

• High school diploma and active Wisconsin CNA certification required.
• CPR certification preferred.
• Valid Wisconsin driver's license with reliable transportation.

Responsibilities

• Care for seniors in their homes and assist with daily activities.
• Help with bathing, toileting, dressing, grooming, nutrition and mobility.
• Assist with ambulation, transfers and range of motion exercises.
• Check vitals, track intake/output, monitor glucose and document changes.
• Collaborate with Hospice nurses and aides to follow care plans.
• Notify supervisor of issues requiring changes in care, medication or equipment.

As a Home Health Certified Nursing Assistant, here’s a big-picture view of what you’ll do:

• Care for seniors and those with an illness, injury or chronic disease in their homes
• Help with daily activities such as bathing, toileting, dressing, grooming, hygiene, nail/skin care, eating and nutritional intake
• Assist with ambulation, transfers and range of motion exercises
• Check vitals, measure intake/output, monitor blood glucose and document any changes
• Work with a team of Hospice nurses and aides to ensure a patient’s care plan is followed
• Observe patient and notify supervisor of issues requiring a change in care, medication or medical equipment

A few must-haves for our Home Health Certified Nursing Assistants:

• High school diploma (or equivalent) and active Nursing Assistant Certification in Wisconsin
• CPR certification
• Valid Wisconsin driver’s license, auto insurance and transportation
• Compassionate nature, good communicator and ability to lift 50 lbs.
