Job summary

Interim HealthCare of Western Wisconsin is seeking a Home Health Certified Nursing Assistant (CNA) for the Eau Claire area. This part-time role focuses on compassionate, one-on-one in-home care for seniors and others in need, with a day-shift schedule Monday through Friday and no nights or weekends.

You will assist with daily activities, monitor vitals, and work with a hospice team to implement each patient’s plan.

Qualifications

• High school diploma (or equivalent) and active Wisconsin CNA certification.
• CPR certification is required.
• Valid Wisconsin driver’s license, auto insurance and transportation are preferred.

Responsibilities

• Care for seniors and those with an illness, injury or chronic disease in patients' homes.
• Assist with daily activities such as bathing, toileting, dressing, grooming, hygiene, nutrition.
• Assist with ambulation, transfers and range of motion exercises.
• Check vitals, monitor intake/output and document changes.
• Collaborate with Hospice nurses and aides to follow the patient’s care plan.
• Observe patients and notify supervisor if changes in care, medication or equipment are needed.
