Job summary

Interim HealthCare of Western Wisconsin is seeking a Home Health Certified Nursing Assistant (CNA) to provide in-home care in Hudson, WI. This part-time role offers daytime shifts Monday through Friday with no nights or weekends, and includes mileage reimbursement for travel between patient visits.

You will assist with daily activities, monitor vitals, and work with a Hospice team to follow each patient’s care plan.

Qualifications

• High school diploma (or equivalent) and active Wisconsin CNA certification.
• CPR certification required.
• Valid Wisconsin driver’s license, auto insurance and reliable transportation.

Responsibilities

• Care for seniors in patients' homes and assist with daily activities such as bathing, dressing, grooming and nutrition.
• Assist with ambulation, transfers and range of motion exercises.
• Check vitals, monitor intake/output and document changes.
• Collaborate with Hospice nurses and aides to follow the patient care plan.
• Observe patient conditions and notify supervisor of any changes affecting care, meds or equipment.

As a Home Health Certified Nursing Assistant, here’s a big-picture view of what you’ll do:

• Care for seniors and those with an illness, injury or chronic disease in their homes
• Help with daily activities such as bathing, toileting, dressing, grooming, hygiene, nail/skin care, eating and nutritional intake
• Assist with ambulation, transfers and range of motion exercises
• Check vitals, measure intake/output, monitor blood glucose and document any changes
• Work with a team of Hospice nurses and aides to ensure a patient’s care plan is followed
• Observe patient and notify supervisor of issues requiring a change in care, medication or medical equipment

A few must-haves for our Home Health Certified Nursing Assistants:

• High school diploma (or equivalent) and active Nursing Assistant Certification in Wisconsin
• CPR certification
• Valid Wisconsin driver’s license, auto insurance and transportation
• Compassionate nature, good communicator and ability to lift 50 lbs.
