Job summary

Team Select Home Care of Colorado, LLC seeks an experienced RN Case Manager to supervise and coordinate skilled nursing care delivered in patients' homes. You will blend hands-on nursing with case management to ensure comprehensive, high-quality home health services aligned with physician orders and regulatory requirements.

You will develop care plans, perform patient evaluations, and coordinate with families, caregivers, and medical professionals.

Qualifications

• RN license required or eligible in applicable state.
• One year nursing experience; home care preferred.
• Ability to supervise ancillary disciplines per state requirements.
• Excellent problem-solving and interpersonal communication skills.

Responsibilities

• Administers and oversees skilled nursing care in patients' homes per physician orders and plan of care.
• Develops and evaluates individualized, patient-centered care plans under physician supervision.
• Performs data collection and documentation (OASIS, PAR, etc.) within required timeframes.
• Acts as advocate, coordinating care among patients, families, agency, and providers.
• Provides medication administration and reconciliation according to policy and orders.
• Conducts on-site evaluations and participates in case conferences as assigned.
