Job summary

Memorial Health is seeking a Patient Care Facilitator to manage care coordination from admission through discharge on an assigned nursing unit. You will work with the interdisciplinary care team to ensure a tailored plan of care and ongoing updates to patients and families regarding expected outcomes.

The role requires BSN or active BSN enrollment, current Illinois RN licensure, CPR certification, and a minimum of two years in acute care.

Qualifications

• BSN or actively enrolled in a BSN program with completion within 2 years.
• Current RN license in Illinois and CPR certification.
• Minimum of 2 years of acute care nursing experience.
• Strong communication and collaboration skills.
• Ability to develop and revise care plans and discharge plans.

Responsibilities

• Coordinate care from admission to discharge for assigned patients.
• Develop and implement individualized care plans with the care team.
• Review and revise plans based on patient progress.
• Communicate with patients and families about expected outcomes.
• Identify and remove barriers to timely goals and plan post-acute care.
