Job summary

Memorial Health is seeking a Patient Care Facilitator to oversee care coordination from admission to discharge on an assigned unit. You will work with the care team to develop and revise plans of care based on patient progress and communicate with patients and families about outcomes and discharge needs.

The role requires an active RN license in Illinois, CPR certification, and a BSN or enrollment in a BSN program with expected completion within 2 years.

Qualifications

• Active participation in unit/patient performance improvement efforts.
• Understanding of performance improvement, care coordination, care transitions, discharge planning, and utilization review.
• Excellent communication, collaboration, and conflict management skills.

Responsibilities

• Coordinate care from admission to discharge for assigned patients.
• Develop and revise individualized plans of care with the interdisciplinary team.
