First Choice Home Health – Mesa, Arizona is seeking a Home Health Care Coordinator to help keep patient care organized, schedules accurate, and communication moving smoothly between patients, clinicians, physicians, and our office team.

This is a great fit for someone who is organized, dependable, detail-oriented, and enjoys helping a healthcare team stay coordinated and on track. The position is in-person at our Mesa office with a part-time schedule and daytime hours.

Responsibilities

• Coordinate patient visits with clinicians, patients, and caregivers.
• Maintain schedules and ensure visits follow physician orders and plans of care.
• Verify insurance eligibility and coverage information.
• Facilitate communication between patients, clinicians, and office staff.
• Follow up with physician offices on outstanding orders and signatures.
• Help resolve scheduling and coordination needs.
• Maintain accurate patient and scheduling information.
• Communicate changes or outstanding needs to the team.
• Support day-to-day home health operations.

Pay: $21.00–$25.00 per hour

Preferred

• Home health, hospice, medical office, or healthcare coordination experience
• Experience scheduling healthcare clinicians
• Insurance eligibility or benefit-verification experience
• Familiarity with physician orders and healthcare documentation
• Experience using an electronic medical record (EMR) system

What You’ll Do
