First Choice Home Health is seeking a Part-Time Home Health Care Coordinator in Mesa, AZ to organize patient care, maintain schedules, and ensure smooth communication among patients, clinicians, physicians, and our office team.

The role emphasizes in-person coordination, attention to detail, and reliable teamwork. Daytime, Mon-Fri hours with a 20-25 hour weekly schedule offer consistent part-time work in-office.

Responsabilités

• Coordinate patient visits with clinicians, patients, and caregivers.
• Maintain schedules and help ensure visits follow physician orders and plans of care.
• Verify insurance eligibility and coverage information.
• Facilitate communication between patients, clinicians, and office staff.
• Follow up with physician offices on outstanding orders and signatures.
• Help resolve scheduling and coordination needs.
• Maintain accurate patient and scheduling information.
• Communicate changes or outstanding needs to the appropriate team members.
• Support day-to-day home health operations.

First Choice Home Health is hiring a Home Health Care Coordinator to help keep patient care organized, schedules accurate, and communication moving smoothly between patients, clinicians, physicians, and our office team.

This is a great fit for someone who is organized, dependable, detail-oriented, and enjoys helping a healthcare team stay coordinated and on track.

Qualifications

Required:

• Strong organization and communication skills
• Ability to manage details and follow tasks through to completion
• Comfortable communicating with patients, clinicians, physician offices, and insurance companies
• Strong computer and administrative skills
• Dependable and team-oriented
• Ability to work in person at our Mesa office

Preferred:

• Home health, hospice, medical office, or healthcare coordination experience
• Experience scheduling healthcare clinicians
• Insurance eligibility or benefit-verification experience
• Familiarity with physician orders and healthcare documentation
• Experience using an electronic medical record (EMR) system
