Job summary

Innovive Health is seeking a Case Manager Registered Nurse to join our team in Massachusetts. The role focuses on coordinating skilled nursing care for home health patients, starting shifts at 7:00 AM, and working under physician orders with clinical oversight.

You will assess patient conditions, revise the care plan, manage medications, educate families, and supervise aides to ensure high-quality care while maintaining HIPAA and documentation standards.

Qualifications

• Graduate of an accredited school of professional nursing.
• Licensed RN by State Board of Nursing (MA).
• Has passed the NCLEX.

Responsibilities

• Provide direct care and case management for home care patients per physician orders.
• Develop and update plan of care and coordinate medications and treatments.
• Assess patient status, adjust plan of care, and communicate with the care team.
• Educate patient and family on self-care, medications, and nutrition.
• Supervise Home Health Aide and LPN and ensure quality of care.
• Maintain HIPAA compliance and accurate clinical documentation.
