Job summary

Innovive Health in Massachusetts is seeking an experienced registered nurse to provide direct patient care in a home health setting. You will assess, develop care plans, coordinate with the clinical team, administer medications, and document visits in our systems.

The role emphasizes patient education, care coordination, HIPAA-compliant documentation, and supervision of aides and LPNs while maintaining high standards of safety and quality care.

Qualifications

• Active RN license required.
• BSN preferred;Graduate of an accredited nursing program.
• Home health or related behavioral health clinical experience.
• Strong communication, organization, and critical thinking skills.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide direct patient care, incl. assessments and care plan development.
• Manage a caseload and coordinate care across the clinical team.
• Administer medications and treatments per physician orders.
• Monitor patient progress and adjust care plans as needed.
• Educate patients and caregivers on care and self-management.
• Communicate with physicians and care teams to ensure continuity of care.
• Supervise and support HHAs and LPNs.
• Complete accurate and timely documentation (OASIS, visit notes, orders).
• Ensure compliance with policies, regulations, and HIPAA standards.
