Job summary

Innovive Health in Gloucester, MA seeks an experienced Registered Nurse Case Manager to deliver and coordinate skilled nursing care at patients' homes. You will work with the Clinical Director and care team to implement physician orders under policy and regulatory standards.

Responsibilities include initial assessments, care planning, medication management, and teaching patients and families. A valid RN license, NCLEX, CPR, and 1–2 years in home health preferred.

Qualifications

• Graduate of an accredited school of professional nursing.
• RN licensed by State Board; BSN with 1 year of related experience or Diploma/ADN with 2 years of related experience.
• Has passed the NCLEX.
• Maintains CPR certification.
• Pre-employment physician health clearance incl. TB skin test and/or CXR.
• Satisfactory references from nursing school or prior/current employers.

Responsibilities

• Provides direct care and case management of assigned patients, including initial assessment and development of plan of care.
• Maintains and updates schedules of all disciplines in patient's home.
• Assesses and evaluates patient status and revises the plan of care as needed.
• Communicates plan of care adjustments to the team to ensure consistent delivery of care.
• Monitors and manages patient medications and coordinates refills with physicians/pharmacies.
• Educates patient and caregiver on self-care techniques.
• Ensures a safe environment and HIPAA compliance.
• Participates in interdisciplinary team meetings and care coordination.
