Job summary

Innovive Health in Lowell, MA is seeking a dedicated registered nurse to provide direct patient care in a home health setting. You will assess patients, develop care plans, manage caseloads, and coordinate with the clinical team to ensure high-quality, compliant care across visits.

You will administer physician-ordered medications and treatments, monitor progress, educate patients and caregivers, and document accurately in line with HIPAA standards.

Qualifications

• Active RN license is required.
• Graduate of an accredited nursing program; BSN preferred.
• Home health or related behavioral health clinical experience.
• Strong communication, organization and critical thinking skills.
• Current CPR certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provide direct patient care, including assessments and care plan development.
• Manage a caseload and coordinate care across the clinical team.
• Administer medications and treatments per physician orders.
• Monitor patient progress and adjust care plans as needed.
• Educate patients and caregivers on care and self-management.
• Communicate with physicians and care teams to ensure continuity of care.
• Supervise and support HHAs and LPNs.
• Complete accurate and timely documentation (OASIS, visit notes, orders).
• Ensure compliance with policies, regulations, and HIPAA standards.
