Job summary

Innovive Health is seeking a Case Manager RN to partner with the Clinical Director and care team to coordinate skilled nursing services for home-based patients in Massachusetts. The role emphasizes patient assessment, plan of care development, medication oversight, and family education.

Ideal candidates will hold an MA RN license and a Bachelor’s in Nursing with NCLEX passed, CPR certified, and preferred home health experience.

Qualifications

• Must hold MA RN license and NCLEX

• Strong ability to coordinate care and communicate with physicians and team

Responsibilities

• Provide direct nursing care and case management for home health patients.
• Maintain and update patient care schedules across disciplines.
• Assess patient status and revise plans of care as needed.
• Communicate care plan changes to the care team promptly.
• Coordinate prescriptions, medications, and refills with providers and pharmacies.
• Educate patient and family on self-care techniques and safety.
• Ensure HIPAA compliance and maintain patient confidentiality.
