Job summary

Innovive Health in Massachusetts is seeking an experienced Case Manager RN to partner with the Clinical Director and team in coordinating skilled home care services. You will assess patients, develop plans of care, oversee medications, and communicate with physicians and providers to ensure quality outcomes.

The role requires an active RN license, BSN preferred, NCLEX passed, CPR certified, and a valid driver’s license, with patient safety and HIPAA compliance at the forefront.

Qualifications

• RN with Massachusetts license and NCLEX certification.
• Bachelor’s degree in nursing or an equivalent diploma/associate with experience.
• Experience in home health and patient care planning involved.
• CPR certification maintained and current.

Responsibilities

• Provides direct care and case management of assigned patients, including initial assessment and plan of care development.
• Maintains and updates schedules of all disciplines in patient’s home.
• Oversees medications and treatments per physician orders and safety standards.
• Communicates plan-of-care changes to the care team to ensure continuity.
• Performs field supervision of care team and ensures HIPAA compliance.
