Job summary

Nuclear Care Partners is seeking a Home Health Aide in Espanola, NM to provide compassionate, one-on-one in-home care to clients in the surrounding areas.

You will assist with ADLs, offer companionship and social engagement, help with meals and light housekeeping, and document changes in EMR per care plans. CPR certification and reliable transportation are required.

Qualifications

• CPR certification required or preferred.
• Ability to work independently in a non-facility setting.
• Valid driver's license and reliable transportation.
• Home health, long-term care, or hospice experience preferred.

Responsibilities

• Assist with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and mobility.
• Provide companionship, emotional support, and social engagement.
• Support meal preparation, nutrition, and light housekeeping.
• Monitor and report changes in condition.
• Follow care plans and document accurately in EMR.
