Job summary

Nuclear Care Partners is seeking a Home Health Aide (HHA) to deliver compassionate, one-on-one care in patients’ homes in Santa Fe, NM. You will support activities of daily living, provide companionship, and help with meals, mobility, and safety, following care plans and documenting changes using EMR standards.

Ideal candidates have a high school diploma or GED, CPR certification, a valid driver’s license with reliable transportation, and the ability to work independently in non-facility

Qualifications

• High school diploma or GED required.
• CPR certification required.
• Valid driver's license and reliable transportation.
• Ability to work independently in a non-facility setting.
• Home health, long-term care, or hospice experience preferred.

Responsibilities

• Assist with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, mobility.
• Provide companionship, emotional support, and social engagement.
• Support meal preparation, nutrition support and light housekeeping.
• Monitor and report changes in condition.
• Follow care plans and document accurately using EMR standards.

Compensation

Compensation is based on experience, shift type, and market factors. The range reflects entry-level to highly experienced applicants.
