Job summary

Nuclear Care Partners is seeking a Home Health Aide (HHA) to deliver compassionate in-home care in Pojoaque, NM, supporting patients with daily living activities and ensuring comfort and safety.

You will work under care plans, document using EMR, provide companionship, monitor changes, and collaborate with the care team to improve quality of life for those who served our country.

Qualifications

• High school diploma or GED required.
• CPR certification required.
• Valid Driver's License and reliable transportation.
• Ability to work independently in a non-facility setting.
• Home health, long-term care, or hospice experience preferred.

Responsibilities

• Assist with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, mobility.
• Provide companionship, emotional support, and social engagement.
• Support meal preparation, nutrition, and light housekeeping.
• Monitor and report changes in condition.
• Follow care plans and document accurately using EMR standards.

Compensation

Compensation is based on experience, shift type, and market factors. The range reflects entry-level to highly experienced applicants.
