Job summary

Muir Home Health is seeking a focused Home Health Aide to join our team in the East Bay, CA. This role provides essential in-home care under supervision of a nurse or therapist and includes documenting observations and care performed to support patient well-being.

Qualifications

• Active Home Health Aide Certification through CA Department of Public Health.
• Current CPR certification.
• Home Health experience preferred.
• Must be a licensed driver with insured vehicle.

Responsibilities

• Provide personal care including baths, back rubs, and oral hygiene.
• Shampoos and nail & skin care to prevent breakdown; assist dressing.
• Assist with toileting, housekeeping, meal planning and feeding.
• Monitor patient condition and report changes to supervising nurse.
• Assist in ambulation, range of motion and basic therapies as ordered.

Pay Range: $24.00 - $28.00/hr

Job Type: Full Time, Part Time, Per Diem

JOB SUMMARY

The Home Health Aide is a paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist and performs various personal care services as necessary to meet the patient’s needs. The home health aide is responsible for observing patients, reporting these observations and documenting observations and care performed.

DUTIES AND RESPSONSIBILITY

Provide personal care including:

• Baths
• Back rubs
• Oral hygiene
• Shampoos
• Change bed linen
• Assist patients with dressing and undressing
• Nail & Skin care to prevent breakdown
• Assist the patient with toileting activities
• Keep patient’s living area clean and orderly

Other duties include:

• Plan and prepare nutritious meals.
• Assist in feeding the patient
• Take and record oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered.
• Assist in ambulation and exercise according to the plan of care.
• Perform range of motion and other simple procedures as an extensional therapy service as ordered.
• Assist patient in the self-administration of medication.
• Do patient’s laundry
• Meet safety needs of patients and using equipment safely and properly (foot stools,
  side rails, etc.)
• Report on patient’s condition and significant changes to the assigned nurse

JOB REQUIREMENTS

• Active Home Health Aide Certification through CA Department of Public Health
• Maintain a current CPR certification
• Home Health experience preferred
• Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
