Job summary

VitaCare Home Health Agency, Inc. is seeking a Home Health Aide for a full-time, on-site role in Trevose, PA. You will assist clients in their homes with daily living activities, promoting dignity and independence, and supporting mobility and safety.

Day-to-day duties include personal care, light housekeeping, meal preparation, and companionship. You will follow care plans, document progress, and collaborate with nurses, therapists, and other team members to ensure consistent, high-quality care.

Qualifications

• Demonstrated skills in Home Care, including in-home support and adherence to care plans.
• Strong Communication skills, including clear, respectful interaction with clients, families, and care team members.
• Ability to assist with Activities of Daily Living such as bathing, dressing, grooming, toileting, and mobility.
• Experience with Patient Care, including observation, reporting changes, and following safety and infection-control practices.
• Proficiency in Personal Care, including hygiene, skin care, and support with continence needs.
• Ability to lift, transfer, and position clients safely, and to perform physically demanding tasks.
• Reliable transportation to client homes in Trevose and surrounding service areas.
• Respect for client privacy, cultural sensitivity, and commitment to non-discriminatory, person-centered care; bilingual skills are a plus.

Role Description

The Home Health Aide role is a full-time, on-site position based in Trevose, PA.
