Job summary

Sweetwater Care is seeking a dedicated Home Health Aide to work under a nurse and provide personalized care according to an individual plan. You will record health status and report to the supervising clinician, while offering companionship and helping patients maintain independence.

The role includes assisting with daily living activities, mobility, meals, light housekeeping, and transportation. Training and adherence to policies ensure high-quality, respectful home care for clients.

Qualifications

• Current and valid Home Health Aide certification.
• CPR certification.
• Ability to lift at least 50 pounds.
• Availability to work on weekends, evenings, and holidays.
• Reliable transportation and valid driver’s license.
• Prior home health experience preferred.
• Participate in facility and company required trainings, in-services and conferences.
• Able to supervise and support physical therapist assistants; rehabilitation aides and/or students.
• Adherence to Company required policy and procedures and code of conduct.
