Job summary

Sweetwater Care in Torrance, CA is seeking a compassionate Home Health Aide to join our care team and support patients in their homes. You'll work with a registered nurse to implement individualized care plans, monitor health status, provide companionship, assist with activities of daily living, and help with mobility and transfers.

Requirements include HHA certification, CPR, ability to lift 50 pounds, valid driver's license and insurance, and willingness to work weekends/evenings/holidays;

Qualifications

• Must hold a valid Home Health Aide certification.
• CPR certification required.
• Ability to lift at least 50 pounds.
• Weekend, evening and holiday availability.
• Reliable transportation.
• Valid driver’s license and insurance.
• Prior home health experience preferred.
• Participate in trainings and in-services.
• Able to supervise rehabilitation staff and students.
• Adherence to company policy and code of conduct.

Responsibilities and Duties

• Work with a registered nurse and other clinicians to provide care based on an individualized plan of care.
• Record your patient's health status and report it to the nurse.
• Provide companionship and personalized care while encouraging independence and self-care.
• Assist patients with activities of daily living, including dressing, bathing, grooming, toileting, and eating.
• Provide mobility and transfer assistance, which may include using mechanical lift devices.
• Prepare meals and snacks. May include grocery shopping.
• Perform light housekeeping, including vacuuming, laundry, and linen changes.
• Assist with incontinence care.
• Assist with transportation needs.

Qualifications and Skills

• Current and valid Home Health Aide certification
• CPR certification
• Ability to lift at least 50 pounds
• Availability to work on weekends, evenings, and holidays
• Reliable transportation
• Valid driver's license and insurance
• Prior home health experience preferred
• Participate in facility and company required trainings, in-services and conferences.
• Able to supervise and support physical therapist assistants; rehabilitation aides and/or students.
• Adherence to Company required policy and procedures and code of conduct.
