Job summary

Sweetwater Care is seeking a compassionate Home Health Aide to work with a registered nurse and other clinicians to implement an individualized plan of care in patients' homes. You will record health status and report to the nurse, provide companionship and personalized care while encouraging independence, and assist with activities of daily living.

You may also assist with meals, mobility and transfers, light housekeeping, incontinence care, and transportation.

Qualifications

• Current and valid Home Health Aide certification.
• CPR certification.
• Ability to lift at least 50 pounds.
• Availability to work on weekends, evenings, and holidays.
• Reliable transportation.
• Valid driver's license and insurance.
• Prior home health experience preferred.
• Participate in facility and company required trainings, in-services and conferences.
• Able to supervise and support physical therapist assistants; rehabilitation aides and/or students.
• Adherence to Company required policy and procedures and code of conduct.
