Job summary

Sweetwater Care in Los Angeles is seeking a dedicated Home Health Aide to work with a registered nurse to implement an individualized plan of care for clients at home. Responsibilities include recording patient status, providing companionship, assisting with daily living activities and mobility, preparing meals, light housekeeping, and supporting transportation needs.

Weekend and evening availability is expected, with CPR and HHA certification required.

Qualifications

• Must hold a current and valid Home Health Aide certification.
• CPR certification required.
• Ability to lift at least 50 pounds.
• Availability to work weekends, evenings, and holidays.
• Reliable transportation and a valid driver\'s license with insurance.
• Prior home health experience preferred.
• Participation in required trainings and in-services.
• Ability to supervise rehab staff and students as directed.
• Adherence to company policies and code of conduct.

• Work with a registered nurse and other clinicians to provide care based on an individualized plan of care.
• Record your patient's health status and report it to the nurse.
• Provide companionship and personalized care while encouraging independence and self-care.
• Assist patients with activities of daily living, including dressing, bathing, grooming, toileting, and eating.
• Provide mobility and transfer assistance, which may include using mechanical lift devices.
• Prepare meals and snacks. May include grocery shopping.
• Perform light housekeeping, including vacuuming, laundry, and linen changes.
• Assist with incontinence care.
• Assist with transportation needs.

Qualifications and Skills

• Current and valid Home Health Aide certification
• CPR certification
• Ability to lift at least 50 pounds
• Availability to work on weekends, evenings, and holidays
• Reliable transportation
• Valid driver's license and insurance
• Prior home health experience preferred
• Participate in facility and company required trainings, in-services and conferences.
• Able to supervise and support physical therapist assistants; rehabilitation aides and/or students.
• Adherence to Company required policy and procedures and code of conduct.
