Buckeye State Health and Family Services is seeking a dedicated Home Health Aide to provide compassionate in-home care. You will assist patients with daily living activities, monitor comfort, and promote independence in a safe, supportive environment.

Join our team to collaborate with nurses and therapists, document progress, respond to medical needs with CPR-certified knowledge, and adapt care plans for seniors and individuals with disabilities.

Qualifications

• Experience in home health care or long-term care environments.
• Strong observation skills to detect changes in condition.
• Experience working with individuals with disabilities or in assisted living settings.
• Knowledge of medical terminology and CPR certification.

Responsibilities

• Assist patients with ADLs including bathing, dressing, grooming, toileting, and mobility with assistive devices.
• Support patients with Alzheimer's, dementia, memory care, hospice, catheter care.
• Perform light housekeeping, meal prep, and maintain a safe home environment.
• Document patient progress and communicate changes with healthcare teams.
• Use medical terminology to record observations clearly for professionals.

Responsibilities

• Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts or transfer belts.

• Support patients with specialized care needs such as Alzheimer's disease, dementia, memory care, hospice care, catheter care, and working with individuals with developmental disabilities or disabilities.

• Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment that meets health standards.

• Document patient progress and communicate effectively with healthcare teams regarding changes in condition or care plans.

• Utilize medical terminology confidently when recording observations or communicating with medical professionals to ensure clarity and accuracy.

Skills

• Proven experience in home health care or long-term care environments with knowledge of medical procedures and documentation practices.

• Strong patient observation skills to detect changes in condition promptly and accurately.

• Experience working with individuals with disabilities or in assisted living settings is highly valued.
