Job summary

Buckeye State Health and Family Services is seeking a compassionate Home Health Aide to support patients at home and in community settings. You will assist with daily living activities, monitor health changes, and collaborate with a care team to promote comfort and independence.

The ideal candidate has experience in home health or long-term care, strong observation skills, and knowledge of medical terminology.

Qualifications

• Proven experience in home health care or long-term care environments with knowledge of medical procedures and documentation practices.
• Strong patient observation skills to detect changes in condition promptly and accurately.
• Experience working with individuals with disabilities or in assisted living settings is highly valued.
• Knowledge of medical terminology along with CPR certification to respond effectively in emergencies.

Responsibilities

• Assist patients with ADLs (bathing, dressing, grooming, toileting, and mobility).
• Support patients with specialized care needs such as Alzheimer’s, dementia, memory care, hospice, catheter care.
• Perform light housekeeping, meal preparation, and maintain a safe living environment.
• Document patient progress and communicate changes with healthcare teams regarding condition or care plans.
• Utilize medical terminology when recording observations to ensure clarity and accuracy.

Responsibilities

• Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts or transfer belts.
• Support patients with specialized care needs such as Alzheimer’s disease, dementia, memory care, hospice care, catheter care, and working with individuals with developmental disabilities or disabilities.
• Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment that meets health standards.
• Document patient progress and communicate effectively with healthcare teams regarding changes in condition or care plans.
• Utilize medical terminology confidently when recording observations or communicating with medical professionals to ensure clarity and accuracy.
