Buckeye State Health and Family Services is hiring a Home Health Aide to provide compassionate in-home care to patients. You will assist with ADLs, monitor conditions, and coordinate with healthcare teams to support independence and safety in the home.

Qualifications

Responsibilities

• Perform light housekeeping, meal preparation, and maintain a safe home environment.
• Document patient progress and communicate changes with healthcare teams.

Responsibilities

• Assist patients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support using equipment like Hoyer lifts or transfer belts.

• Support patients with specialized care needs such as Alzheimer's disease, dementia, memory care, hospice care, catheter care, and working with individuals with developmental disabilities or disabilities.

• Perform light housekeeping duties including cleaning, laundry, meal preparation, and ensuring a safe living environment that meets health standards.

• Document patient progress and communicate effectively with healthcare teams regarding changes in condition or care plans.

• Utilize medical terminology confidently when recording observations or communicating with medical professionals to ensure clarity and accuracy.
