Job summary

Buckeye State Health and Family Services is seeking a compassionate Home Health Aide to support patients at home and in community settings. You will assist with daily living activities, monitor health changes, and collaborate with a care team to promote comfort and independence.

The ideal candidate has experience in home health or long-term care, strong observation skills, and knowledge of medical terminology.

Qualifications

Proven experience in home health care or long-term care environments with knowledge of medical procedures and documentation practices.
Strong patient observation skills to detect changes in condition promptly and accurately.
Experience working with individuals with disabilities or in assisted living settings is highly valued.
Knowledge of medical terminology along with CPR certification to respond effectively in emergencies.

Responsibilities

Assist patients with ADLs (bathing, dressing, grooming, toileting, and mobility).
Support patients with specialized care needs such as Alzheimer’s, dementia, memory care, hospice, catheter care.
Perform light housekeeping, meal preparation, and maintain a safe living environment.
Document patient progress and communicate changes with healthcare teams regarding condition or care plans.
Utilize medical terminology when recording observations to ensure clarity and accuracy.
