Qualifications

Active CNA certificate required.
Valid driver's license and car insurance.
CPR certification.
Experience in home health is a plus.
Familiarity with EMR Kinnser by WellSky is advantageous.

Responsibilities

Assist with Activities of Daily Living (ADLs): bathing, dressing, grooming, and toileting for home health patients.
Mobility support: assist with walking, transferring, and mobility aids.
Vital signs: take and record blood pressure, pulse, and temperature.
Medication reminders and ensure adherence to schedules.
Nutrition assistance: meal preparation and monitoring dietary restrictions.
Hygiene and comfort: maintain hygiene and comfort, change bed linens, oral care.
Observation and reporting: note changes in condition to nursing team.
Documentation: document care and vitals in EMR (Kinnser by WellSky).
Communication: maintain clear communication with patient, family, and healthcare team.
Safety: follow infection control and safety protocols.
Transportation: provide transportation to appointments as needed.
Emotional support and companionship to patients.
Collaboration: work with nurses and therapists.
Patient education on self-care and safety measures.
