Qualifications

Responsibilities

• Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting.
• Support mobility including walking, transferring, and use of mobility aids.
• Take and record vital signs and monitor health status.
• Provide medication reminders and ensure adherence to schedules.
• Prepare meals and assist with feeding according to dietary restrictions.
• Maintain hygiene and comfort; change linens and provide oral care.
• Document care provided and changes in patient condition in EMR.
• Communicate clearly with patients, families, and healthcare team.
• Follow infection control and safety protocols; transport to appointments as needed.
• Offer emotional support and companionship to patients.

Responsibilities

• Vital Sign Monitoring: Take and record vital signs, including blood pressure, pulse, and temperature.

• Nutrition Assistance: Help with meal preparation, feeding, and monitoring dietary restrictions as prescribed.
• Hygiene and Comfort: Maintain patient hygiene and comfort, including changing bed linens and providing oral care.
• Observation and Reporting: Observe and report changes in the patient's condition, behavior, or symptoms to the nursing team.
• Documentation: Accurately document care provided, vital signs, and any changes in the patient's condition in the EMR system (Kinnser by WellSky).

Qualification
