Elite Care Management, Inc. is seeking Certified Nursing Assistants for full-time, part-time, and PRN shifts across multiple Illinois locations. You’ll provide one-on-one in-home care, assist with ADLs, mobility, and daily tasks, and support patients and families throughout shifts.

Qualifications

• Certified Nursing Assistant (CNA) certification required.
• Current CPR certification is mandatory.
• TB test or chest X-ray within the last 12 months.
• Valid driver’s license and auto insurance card.

Responsibilities

• Assist the nurse on duty and monitor patient status.
• Maintain records of patient care, condition, progress, or problems.
• Support mobility by helping patients transfer as needed.
• Assist with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, meals.
• Light housekeeping and meal preparation as part of daily care.
• Engage patients to enhance quality of life and mental alertness.

Responsibilities may include the following:

• Assisting the nurse on duty
• Maintain records of patient care, condition, progress, or problems to report and discuss observations with supervisor or clinical manager
• Support mobility by assisting patients with transferring as needed
• Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, meal prep, and eating
• Light house-keeping duties, such as changing bed linens, laundry, cleaning, or preparing meals
• Enhancing quality of life and building relationships with patients through conversation, doing activities together, and finding stimulation to keep them mentally healthy and alert.
