Safe Life Home Health Care, LLC is seeking dedicated CNAs to join our team in Chicago, IL. As a Home Health Aide/CNA, you will assist patients with activities of daily living, monitor vital signs, manage medications reminders, and provide compassionate care in a home setting.

You will collaborate with nurses and therapists to ensure patient well-being, document care in the EMR (Kinnser by WellSky), and support patients and families with education and safety.

Qualifications

Active CNA certificate required.
Valid driver’s license and car insurance.
CPR certification required.
Familiarity with EMR Kinnser by WellSky is advantageous.
Excellent communication skills necessary.

Responsibilities

Assist with Activities of Daily Living (ADLs) for home health patients.
Support mobility and transfers; assist with walking and mobility aids.
Take and record vital signs (blood pressure, pulse, temperature).
Provide medication reminders and monitor schedules.
Prepare meals and monitor dietary restrictions as prescribed.
Maintain patient hygiene and comfort; change bed linens and oral care.
Document care provided and vital signs in the EMR system (Kinnser by WellSky).
Communicate clearly with patient, family, and healthcare team.
Ensure safety and infection control; follow protocols.

Responsibilities

Assist with Activities of Daily Living (ADLs): Provide assistance with bathing, dressing, grooming, and toileting for home health patients.
Mobility Support: Help patients with mobility by assisting them with walking, transferring, and using mobility aids.
Vital Sign Monitoring: Take and record vital signs, including blood pressure, pulse, and temperature.
Medication Reminders: Assist patients with medication reminders and ensure compliance with medication schedules.
Nutrition Assistance: Help with meal preparation, feeding, and monitoring dietary restrictions as prescribed.
