Job summary

Responsibilities
- Assist with personal care and activities of daily living (ADLs).
- Perform safe transfers and mobility assistance using a Hoyer lift.
- Provide dementia-appropriate communication and support.
- Perform light household tasks related to patient care.
- Monitor client condition and report changes to supervisor.

- 3 twelve‑hour shifts per week
- Rotating schedules
- Day or night availability depending on client needs
- Current CNA license in the State of Illinois
- Reliable transportation and ability to commute to the client's home in Bartlett
- Comfortable working with dementia patients
- Experience using a Hoyer lift
- Dependable, professional, and patient‑focused
- Personal care and ADLs
- Safe transfers using a Hoyer lift
- Dementia‑appropriate communication and support
