Job summary

BAYADA Home Health Care is seeking an experienced Registered Nurse for Home Health Admissions in Northfield, New Jersey. This role may be full-time, part-time, or per diem, supporting our office in Egg Harbor Township and caring for clients across Atlantic County.

The RN Admissions nurse will follow care plans, perform home visits, administer medications, and document care using EMR tools. Travel to cases is required and a NJ RN license is essential.

Qualifications

• NJ RN license required.
• Minimum of one year of clinical experience in medical/surgical care.
• Home health experience preferred but not required.

Responsibilities

• Follow a designated care plan tailored to each patient.
• Perform home visits to clients in assigned geographic territories.
• Administer medications, wound care, and routine treatments.
• Monitor patient status and report changes to supervisors.
• Document orders and clinical observations accurately in the EMR.
• Coordinate case management and care transitions.
