Job summary

BAYADA Home Health Care is seeking an Admissions Registered Nurse (RN) for full-time home health admissions and SOC visits across Monmouth, Middlesex, Union, and Somerset Counties in New Jersey. You will perform comprehensive initial assessments and help set up individualized plans of care with the interdisciplinary team.

Requirements include an active NJ RN license, at least one year of clinical nursing experience, and prior home health and OASIS exposure.

Qualifications

• Active New Jersey RN license.
• Minimum 1 year of clinical nursing experience.
• Prior home health experience required.
• OASIS experience required.
• Strong assessment and care coordination skills.
• Excellent communication and time-management skills.
• Ability to travel throughout Monmouth, Middlesex, Union, and Somerset Counties.

Responsibilities

• Complete comprehensive OASIS Start of Care (SOC) admissions visits for home health clients in the territory.
• Perform thorough initial nursing assessments, including medication reconciliation and safety evaluations.
• Develop individualized plans of care in collaboration with physicians, clients, caregivers, and the interdisciplinary team.
• Identify client needs and coordinate nursing, therapy, social work, and other home health services.
• Review and execute physician orders and ensure the plan of care reflects client needs and goals.
• Educate clients and caregivers regarding diagnoses, medications, treatments, safety, and the home health plan of care.
