AHN Healthcare@Home is hiring an Admissions Registered Nurse (RN) for home health in the Wilkinsburg, PA area. The role offers robust bonus potential and opportunities for growth, with a strong emphasis on patient-centered care and independent field work.

We are hiring an Admissions Registered Nurse (RN) for a Home Health position providing skilled nursing care to patients in their homes in the Wilkinsburg, PA area.

This position offers up to $30,000 in total bonus potential over your first three years:

• $10,000 first-year success bonus (Year 1) — rewarding key achievements during your first year
• $5,000 sign-on bonus — paid within your first 12 months
• Up to $500/month bonus eligibility (Year 2) — an additional $6,000 potential
• Up to $750/month bonus eligibility (Year 3) — an additional $9,000 potential
• The monthly bonus grows up to $1000 per month eligibility after 4 years!

To receive these bonuses, you must be in good standing, meet performance expectations, and remain in the role and status for which you were hired!

What You’ll Do:

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Conduct comprehensive start-of-care assessments, including clinical evaluations, medication reconciliation, and development of initial care plans aligned with physician orders
• Educate and guide patients and families through the admission process, setting expectations for care, reviewing diagnoses and treatment plans, and ensuring they feel informed and supported from day one
• Apply strong clinical judgment and critical thinking to coordinate admissions efficiently, address barriers to care, and ensure accurate, timely, and compliant EMR documentation
• Initiate evaluations to other disciplines when indicated
• Work independently, but never alone
• Travel to patient homes within assigned territory

RN, Start of Care Qualifications

• Active Registered Nurse, RN license in the state you work
• One year of RN experience and the clinical expertise to deliver quality patient care
• Due to the field-based nature of this home health position, a valid driver’s license and the ability to spend approximately 20% of your workday driving to and from patient locations are required.
