Job summary

The Home Health Admissions Coordinator (Intake Coordinator) at TheraCare Home Health in Dallas, TX manages the referral-to-admission workflow for home health patients. Qualifications

• High school diploma or equivalent required; healthcare admin education preferred.
• 1–2 years in home health admissions/coordinator roles is desired.
• Experience verifying insurance eligibility for Medicare/Medicaid/commercial plans.
• Experience with EMR systems and Homecare Homebase is preferred.
• Knowledge of Medicare home health rules and face-to-face docs is a plus.
• Strong organization and ability to handle multiple referrals in a fast-paced env.
• Excellent communication with physicians, hospitals, patients, and staff.

Responsibilities

• Manage incoming home health referrals and start admissions process.
• Review referral docs: physician orders, discharge summaries, H&P, face-to-face.
• Coordinate with hospitals, clinics, case managers, and partners for missing papers.
• Enter and maintain patient demographics and referral details in the EMR.
• Perform insurance verification and authorization coordination for payers.
• Coordinate Start of Care visits with clinical leadership and scheduling teams.
• Monitor referral status and ensure timely admissions per regulations.
• Communicate updates with intake staff and leadership.
• Contact patients/caregivers to confirm demographics and coverage.
• Maintain admissions logs and dashboards to track activity and delays.
• Ensure all documentation meets Medicare/Medicaid/payer standards.
