Job summary

Cantex is seeking a Home Health Admissions Coordinator to manage the referral-to-admission workflow for home health patients in Texas. You will coordinate referrals, verify insurance, manage physician documentation, and schedule Start of Care visits to ensure compliant admissions.

The role requires strong organizational skills, effective communication with physicians and hospitals, and proficiency with EMR systems. This on-site position supports a fast-paced home health intake team.

Qualifications

• High school diploma or equivalent required; healthcare administration or related education preferred.
• 1–2 years of experience in home health admissions, intake, or scheduling within healthcare.
• Experience verifying insurance eligibility for Medicare/Medicaid or commercial plans.
• Experience using an EMR system; Home Health EMR preferred.
• Familiarity with Homecare Homebase or similar documentation systems preferred.
• Knowledge of Medicare home health regulations and face-to-face documentation preferred.

Responsibilities

• Manage incoming home health referrals and initiate admissions process.
• Review referral documentation to ensure required records are present.
• Coordinate with hospitals, physicians, case managers, and referral partners to obtain missing documentation.
• Enter and maintain accurate patient demographics and referral details in the EMR.
• Perform insurance verification and eligibility review for Medicare/Medicaid and commercial payers.
• Coordinate Start of Care visits with clinical staff and scheduling teams.
• Monitor referral status and ensure admissions occur within regulatory timeframes.
• Communicate updates with intake staff and leadership.
• Maintain admissions logs, referral trackers, and dashboards.
