Qualifications

• Minimum of three years of postgraduate experience in social work, child welfare, foster care, or related human services.

Responsibilities

• Recruit prospective foster and adoptive families through outreach, events and presentations.
• Complete foster/adoptive home studies, family assessments, and annual evaluations.
• Coordinate medical, behavioral health, educational, and community services for children in care.
• Develop, implement, and regularly update individualized treatment plans.
• Monitor licensing compliance and changes in family circumstances; develop corrective action when needed.
• Maintain regular contact with children and foster families, including home visits.
• Manage assigned caseloads and ensure timely documentation and reporting.

Foster Care Home Evaluator & Treatment Coordinator

Burlington United Methodist Family Services, Inc. d/b/a Star of Hope Family Services is seeking a compassionate, organized Foster Care Home Evaluator & Treatment Coordinator to join our foster care team.

If you are a human services professional who wants to make a difference for children and families, this position offers a unique opportunity to support both sides of foster care—helping recruit, prepare, and support foster families while coordinating the services children need to achieve stability, permanency, and long‑term success.

What You’ll Do

• Recruit prospective foster and adoptive families through community outreach, recruitment events, presentations, and other activities.


• Work toward opening a minimum of five new foster homes annually.


• Conduct initial inquiries and orientations with prospective foster and adoptive families.


• Complete foster and adoptive home studies, family assessments, annual home evaluations, and health and safety reviews.


• Monitor foster homes for licensing compliance and changes in family circumstances.


• Develop and implement corrective action plans when licensing or safety concerns are identified.


• Coordinate and facilitate foster parent pre‑service and ongoing training, including applicable therapeutic foster care curriculum.


• Provide foster families with ongoing consultation, retraining, problem‑solving, and support.


• Manage an assigned caseload of children and foster families.


• Maintain regular contact with children and foster families, including required face‑to‑face and in‑home visits.


• Coordinate medical, behavioral health, educational, and community services for children in care.


• Develop, implement, and regularly update individualized treatment plans.


• Participate in multidisciplinary team meetings, treatment planning, permanency planning, court proceedings, and other case‑related meetings.


• Conduct or supervise family visitation as recommended.


• Assist with child placements and provide ongoing support to promote placement stability.


• Support permanency planning, adoption referrals, and transition services.


• Serve as an emergency resource for foster families and participate in an on‑call rotation.


• Maintain accurate and timely licensing, treatment, case management, billing, and regulatory documentation.


• Complete required reports, referrals, outcome measures, and continuous quality improvement activities.


• Provide written reports and court testimony when required.

What We’re Looking For

Required Education

• A Bachelor's degree in Social Work, Psychology, Counseling, Sociology, Education, or another related human services field from an accredited college or university.

Social Work or other applicable professional licensure is preferred and may be required based on applicable West Virginia regulations.

A minimum of three years of postgraduate experience in social work, child welfare, foster care, or a related human services field is preferred.

• Can manage multiple cases, deadlines, appointments, and regulatory requirements.


• Works independently while remaining highly collaborative with the broader treatment team.


• Demonstrates confidentiality, sound judgment, accountability, respect, empathy, and professionalism.


• Is comfortable traveling extensively to provide services to children and families.

More Than Case Management

Foster care is about more than finding a placement. It is about creating the support system children and families need to move toward safety, stability, healing, and permanency.

In this role, you will have the opportunity to help identify and prepare families who can provide safe, supportive homes while also remaining actively involved after placement. From foster parent training and home evaluations to treatment planning, family support, and permanency services, your work will directly influence the experience of children and families throughout their foster care journey.

Additional Requirements

• Be at least 21 years of age.


• Possess a valid state‑issued driver's license.


• Maintain an acceptable driving record and meet Agency requirements for operating Agency‑owned or leased vehicles.


• Be willing and able to travel extensively as required to serve children and families.


• Be available to participate in an on‑call rotation and respond to emergency needs involving foster families.


• Successfully complete all required background investigations and employment screenings.


• Maintain all required training, licensing, and regulatory qualifications applicable to the position.

The position requires flexibility to accommodate home visits, family needs, court proceedings, community activities, emergencies, and other responsibilities that may occur outside traditional office hours.
