Qualifications

  • Knowledge of child welfare principles, licensing standards, and applicable federal and state regulations.
  • Strong assessment, interviewing, case management, and treatment planning skills.

The Foster Care Home Evaluator & Treatment Coordinator is responsible for recruiting, evaluating, licensing, training, and supporting foster and adoptive families while providing case management and treatment coordination for children placed in foster care. This position ensures compliance with agency, state, and federal regulations while promoting positive outcomes for children and families through assessment, treatment planning, licensing, support services, and permanency planning.


Essential Duties

Agency Responsibilities


  • Support the mission, vision, values, and policies of Star of Hope Family Services.

  • Demonstrate professionalism, confidentiality, teamwork, respect, and cultural competency.

  • Utilize agency communication systems effectively.

  • Support accreditation, licensing, and regulatory compliance.

  • Participate in professional development and ongoing training.

  • Complete all documentation accurately and within required timelines.

  • Perform other duties as assigned within the scope of the position.


Foster Home Recruitment & Licensing


  • Recruit prospective foster and adoptive families through community outreach and recruitment activities.

  • Open a minimum of five new foster homes annually.

  • Conduct initial inquiries and orientation with prospective foster/adoptive families.

  • Complete foster/adoptive home studies, family assessments, annual home evaluations, and health and safety reviews.

  • Develop and implement corrective action plans related to licensing or safety concerns.

  • Monitor foster homes for changes in family circumstances and licensing compliance.

  • Maintain complete licensing documentation in agency information systems.

  • Organize, coordinate, and facilitate foster parent pre-service and ongoing training, including Pressley Ridge Therapeutic Foster Care curriculum as applicable.

  • Meet individually with foster families for retraining, consultation, or problem resolution.

  • Participate in community recruitment events, open houses, health fairs, and educational presentations.


Foster Care Case Management & Treatment Coordination


  • Manage an assigned caseload of foster children and foster families.

  • Establish and maintain ongoing support systems through regular contacts, including a minimum of two face-to-face visits monthly (one occurring in the child's home) and weekly contact with foster families.

  • Coordinate medical, behavioral health, educational, and community services for children in care.

  • Develop, implement, and update individualized treatment plans every 90 days or as clinically indicated.

  • Participate in Multi-Disciplinary Team (MDT) meetings, treatment planning meetings, permanency planning, and court proceedings when required.

  • Conduct and supervise family visitation as recommended.

  • Assist with child placements and placement stability.

  • Serve as an emergency resource for foster families and participate in the on-call rotation.

  • Support permanency planning, adoption referrals, and transition services.


Documentation & Compliance


  • Maintain timely and accurate documentation in Extended Reach, PATH, and other required electronic systems.

  • Complete monthly progress reports, strategic outcome measures, CQI activities, billing documentation, referrals, and regulatory reports within established deadlines.

  • Complete referrals through required state portals (including KEPRO and Atrezzo) as applicable.

  • Maintain all licensing, treatment, and case management documentation in compliance with state and agency standards.

  • Provide written reports and court testimony when required.

Qualifications

Education


  • Bachelor's degree in Social Work, Psychology, Counseling, Sociology, Education, or another related human services field from an accredited college or university.

  • Social Work or applicable professional licensure is preferred and may be required in accordance with West Virginia regulations.


Experience


  • Minimum of three years of postgraduate experience in social work, child welfare, foster care, or a related field of human services preferred.


Required Knowledge, Skills & Abilities


  • Knowledge of child welfare principles, foster care licensing standards, and applicable federal and state regulations.

  • Strong assessment, interviewing, case management, and treatment planning skills.

  • Excellent written, verbal, organizational, and computer skills.

  • Ability to manage multiple priorities while meeting deadlines.

  • Ability to respond effectively during crisis situations.

  • Strong interpersonal skills with the ability to collaborate with families, courts, schools, providers, and community partners.

  • Ability to prepare thorough reports and maintain accurate records.

  • Ability to work independently while functioning effectively as part of a multidisciplinary team.

  • Commitment to ethical practice and confidentiality.

  • Must be at least 21 years of age.

  • Valid driver's license, acceptable driving record, and ability to travel extensively.
