Lincare Holdings Inc. seeks a field-based healthcare professional to follow up with patients and evaluate adherence to physician orders. You will educate patients, perform assessments, and coordinate care in enteral home care settings.

The role requires interaction with doctors and referral sources, maintaining records, and handling logistics for delivering supplies to patients’ homes. Training and ongoing education are provided as part of the position.

Qualifikationen

• Read and interpret documents such as safety rules, operating instructions, and procedure manuals.
• Calculate figures such as commissions, proportions, percentages, area, circumference, and volume.
• Solve practical problems with a variety of concrete variables where standardization is limited.
• Interpret instructions in written, oral, diagram, or schedule form.
• Knowledge of word processing, spreadsheets, and email communications.

Aufgaben

• Provides patient education as part of patient care as needed and per policy.
• Performs complete and professional patient assessments per policies and procedures.
• Maintains knowledge of enteral home care techniques and concepts.
• Represents the company through professional appearance, patient care activities, and written reports.
• Gathers information from referral sources to ensure insurance coverage for enteral patients.
• Follows up on paperwork to ensure payment for services rendered.
• Maintains patient records in a complete and organized manner per accreditation standards.
• Drives to deliver and pick up enteral supplies to patients' homes or places of business.
• Provide patient care, treatment, and service in a continuous and coordinated manner.
• Promote safe and effective patient and organizational environments and equipment use.
• Participate in ongoing education, in-services, training and other activities to maintain competence.

One year experience as LPN/DTR/CNA preferred
