Job summary

Lincare Holdings is seeking a qualified healthcare professional to provide patient education and comprehensive assessments in home health settings. You will manage enteral care, ensure policy adherence, and coordinate with referral sources for insurance coverage.

The role involves maintaining patient records, driving to patient homes to deliver supplies, and upholding safety in every interaction while supporting ongoing education and competence.

Qualifications

• Minimum one year in home health care, hospital, clinic, or nursing home preferred.
• Experience/education in Dietetics, Nutrition, Home Health, or Nursing preferred.
• One year of experience as an LPN, DTR, or CNA preferred.

Responsibilities

• Provides patient education as part of patient care according to patient needs and policy.
• Performs complete patient assessments per policies and procedures.
• Maintains current knowledge of enteral home care techniques and related concepts.
• Represents the company through professional appearance, patient care activities, and written reports.
• Gathers information from referral sources to ensure insurance coverage.
