Job summary

Responsibilities include maintaining records, coordinating with referrals for insurance coverage, and driving to patient homes to deliver and service equipment.

Qualifications

• One year experience in home health care or related setting preferred.
• Strong interpersonal skills to interface with doctors, referrals, and patients.
• Ability to read and interpret care documents and policies.

Responsibilities

• Provide patient education as part of care and policy.
• Perform complete and professional patient assessments per procedures.
• Maintain knowledge of enteral home care techniques and concepts.
• Represent the company through professional appearance and care activities.
• Gather information from referral sources to ensure insurance coverage and follow up on paperwork.
• Maintain patient records in compliance with accreditation standards.
• Drive to deliver and pick up enteral supplies to patients’ homes or business locations.
• Provide continuous patient care and coordinate services to meet patient needs.
• Promote safe, effective environments and equipment use; participate in ongoing education.
