Qualifications

• One year experience in home health care, hospital, clinic, or nursing home strongly preferred.
• Experience/Education in Dietetics, Nutrition, Home Health, or Nursing strongly preferred.
• Must have excellent human relations skills to interface with doctors, other referral sources, and patients.

Responsibilities

• Provides patient education as part of patient care.
• Performs patient assessments according to policies.
• Admits and delivers enteral supplies to patients' locations.
• Maintains patient records in compliance with standards.
